Overview
All eligible participants are automatically enrolled in Life Insurance and Accidental Death & Dismemberment (AD&D) coverage. You do not need to be enrolled in other benefits to qualify.

Eligibility
Only eligible participants and their dependents, as defined in Article I of the SPD, are covered for Life Insurance benefits. Life Insurance coverage ends when eligibility ends under Article II of the SPD, except that eligibility for Life Insurance does not continue during COBRA continuation coverage. Life Insurance
Upon your death
When proof of your death is received by the Benefit Administrative Office, the Life Insurance benefit will be paid to your beneficiary, in accordance with Article XII of the Summary Plan Description (SPD) found in the Forms & Documents section of the website.
Upon your dependent’s death
When proof of your dependent’s death is received by the Benefit Administrative Office, the Life Insurance benefit will be paid to you (the employee).
Be sure to designate your beneficiary
You may designate one or more beneficiaries to receive the Life Insurance benefit upon your death, or change any beneficiary designation, at any time, by submitting your beneficiary designation notice in the secure member portal. Any designation or change will take effect after it has been received by the Benefit Administrative Office, provided benefits have not been paid before it was received. Additionally, any designation or change of beneficiary must be received by the Benefit Administrative Office before your death to be effective.
What’s covered
Life Insurance benefits are provided directly by the Fund in the amount of $50,000 upon your death or the death of a dependent.
Accidental Death & Dismemberment benefits
Accidental Death & Dismemberment benefits are not available for the accidental death or injury of a dependent. See the Life Insurance section for information on the death of a dependent.
Upon your accidental death
When proof of your accidental death is received by the Benefit Administrative Office, and your accidental death occurs within 90 days after and as a direct result of an accident, the Accidental Death & Dismemberment benefit for loss of life is paid to your named beneficiary or beneficiaries.
Upon your accidental dismemberment
When, within 90 days after and as a direct result of an accidental injury, you sustain one of the losses listed below (other than loss of life), and proof of such loss is received by the Benefit Administrative Office, a dismemberment benefit will be paid to you.
Be sure to designate your beneficiary
You may name or change any beneficiary at any time on the secure member portal. The beneficiary rules set forth in Article XII, under “Beneficiary for Employee Life Insurance” in the SPD (found in the Find Forms & Documents section of the website), apply to payment of the Accidental Death benefit.

For accidental death
Accidental death benefits are provided directly by the Fund in the amount of $20,000 upon your death.
For accidental dismemberment
For any of the following, a $20,000 benefit will be paid:
Both hands or both feet
Sight of both eyes
One hand and one foot
One hand and sight of one eye
One foot and sight of one eye
For the loss of the following, a $10,000 benefit will be paid:
One hand or one foot
Sight of one eye
Only one benefit, in the greater of the amounts payable, will be paid as a result of all injuries or losses sustained in or as the result of any one accident.
All claims must be received by the Benefits Administrative Office within one year of death or accidental injury or the benefits will not be payable.
Definitions
Loss means, with respect to hands and feet, the actual severance at or above the wrist or ankle joint, and with respect to eyes, the entire and irrecoverable loss of sight.
Accidental death means death resulting from an external, violent and unexpected means and includes death by homicide.
Accidental injury means bodily injury resulting from an external, violent, and unexpected means.
