Overview
Dental benefits are available to all eligible employees and their dependents through one of three different dental plans. Your choice of dental plans is determined by your Fund Plan and address. You can log in to the secure member portal to see which dental plans are available to you and your eligible family members or you can contact the Benefit Administrative Office at (866) 345-5189.

Dental Office at the UNITE HERE Health Center
Dental care at the UNITE HERE Health Center is provided by DYNTL exclusively for Fund members and their enrolled dependents. All eligible members have the option to enroll in the UNITE HERE Dental Office. When you come to the Dental Office at the UNITE HERE Health Center for care, you can expect excellent care provided by DYNTL, with no upselling, and a more comfortable experience.

Liberty Dental HMO Plan
The Liberty Dental HMO Plan has very low copays and a wide network of providers.
The Liberty Dental Plan covers only services and procedures listed on the “Schedule of Benefits,” when provided by a participating dental provider identified in the “CA Select” network on the Liberty Dental website. Visit the Liberty Dental Plan: Find A Dentist webpage to learn more.
Services and procedures provided by non-participating dental providers are not covered under the Plan.
Getting started
Begin using your Liberty Dental benefit in three easy steps:
Check the mail for your Liberty Dental insurance card. ID cards will be included in your welcome packet. If you do not receive your card or need additional cards, call the Liberty Dental Call Center at (888) 703-6999.
Create an online account. Visit the Liberty Dental Member Portal and select “Click here to create an account.” Select “Member” as the type of user and then complete the form. You will receive an email to activate your account. You will need your member ID number to create your online account.
Find a dentist. Once you are logged in to your Liberty Dental account you can search for in-network providers. Note: if you do not want to create an online account, you can call (888) 703-6999 to get assistance finding a dentist.
What’s covered

Delta Dental PPO Plan
Delta Dental is available as an option to members who live more than 20 miles from the Dental Office at the UNITE HERE Health Center only—and is not available to members of the UNITE HERE Restaurant Plan. The Delta Dental PPO Plan offers a large network of dentists plus savings on out-of-pocket costs for covered services. Visit the Delta Dental website to learn more.
Getting started
Check the mail for your welcome letter. When you enroll, Delta Dental will send you a welcome letter or email along with your ID card. Your letter will contain your Enrollee ID, which you will need to create an online account. If you do not receive your letter, call Delta Dental at 888-335-8227.
Create an online account. Visit the Delta Dental Member Resources page and click “Your account,” and then “Register”. Select “Enrollee/Adult dependent” from the drop-down menu and enter your personal information, including your Enrollee ID found on your ID card.
Find a dentist. To locate a dentist in the Delta Dental PPO network, log in to your account and select “Find a dentist”. You can also search the online directory on their website by entering your location and selecting “Delta Dental PPO” from the drop-down list.
What’s covered
A summary of benefits under the Delta Dental PPO Plan is available in the Plan Benefits Highlights issued by Delta Dental. Under the Delta Dental PPO Plan, you can visit any licensed dentist. However, your out-of-pocket costs will be lower if you go to a Delta Dental PPO dentist.

The Combined Evidence of Coverage and Disclosure Form and the Delta Dental PPOSM Group Dental Service Contract between the Fund and Delta Dental of California (collectively “Delta Dental Plan Documents”) provide a complete description of benefits under the Delta Dental PPO Plan.
