Dr. Sophie Chang is a board-certified family medicine physician with over 30 years of experience dedicated to delivering comprehensive, patient-centered care for individuals and families of all ages.

Dr. Chang speaks English, Spanish, Mandarin and Taiwanese and believes in providing compassionate, culturally sensitive care that empowers patients to take an active role in their health and well-being, fostering long-term wellness and resilience.

Dr. Chang is skilled in managing a wide range of health conditions and coordinating care across specialties to ensure optimal patient outcomes.

She earned her medical degree from Our Lady of Fatima College of Medicine and completed her residency at the University of Minnesota Medical School Department of Family Medicine and Community Health. Further enhancing her expertise, Dr. Chang completed Primary Care Fellowship in Pain Management at the University of California, Davis, Primary Care Fellowships in Psychiatry, as well as in Addiction Medicine at the University of California, Irvine.

  • Routine immunizations and seasonal vaccinations

  • Minor Surgical Procedures: Removal of skin tags, warts, and other skin lesions; suturing of minor cuts and wounds.

  • Injections: Administration of medications such as antibiotics, steroids, and pain relief.

  • Wound Care: Cleaning, dressing, and management of minor wounds and burns.

  • Ear and Eye Care: Earwax removal, foreign body removal from eyes, and treatment of minor eye infections.

  • On-site Phlebotomy for Blood Tests: Complete blood count (CBC), lipid panel, liver function tests, and thyroid function tests.

  • Urine Tests: Urinalysis for detecting infections, kidney function, and other conditions.

  • Stool Tests: Testing for gastrointestinal infections and conditions.

  • Rapid Diagnostic Tests: Rapid tests for strep throat, influenza, and COVID-19.

  • Screening Tests: Cholesterol screening, diabetes screening, and cancer screening (e.g., PSA for prostate cancer).

  • Behavioral Health Assessments: Initial assessments and referrals for more comprehensive mental health services if needed.

  • Medication Management: Monitoring and management of medications for mental health conditions.

  • Well-Child Visits: Routine check-ups to monitor growth and development, including immunizations and developmental screenings.

  • Acute Care: Treatment for common childhood illnesses such as ear infections, colds, and flu.

  • Chronic Condition Management: Ongoing care for children with chronic conditions such as asthma and diabetes.

  • Behavioral Health: Support for developmental and behavioral issues, including ADHD and anxiety.

  • Gynecological Exams: Routine pelvic exams, Pap smears, and breast exams.

  • Family Planning: Birth control counseling and services, including prescriptions for oral contraceptives, contraceptive injections, IUD insertion, and contraceptive implants.

  • Early Prenatal Care: Includes direct referral to prenatal specialist for comprehensive care during pregnancy, including ultrasounds, blood work, and nutritional counseling.

  • Menopause Management: Support and treatment for symptoms related to menopause.

  • Diabetes Care: Comprehensive management of diabetes, including blood sugar monitoring, medication management, and lifestyle counseling.

  • Heart Disease Management: Monitoring and treatment of heart conditions, including hypertension and cholesterol management.

  • Chronic Lung Disease and Asthma Management: Personalized care plans to manage asthma symptoms and prevent attacks.

  • Weight Management: Support for achieving and maintaining a healthy weight through diet and exercise counseling.
